Workers Compensation

Employees who experience a work-related injury or illness should immediately notify their supervisor and jointly complete the appropriate forms (found on this page), then submit the completed forms to Human Resources.

• Treatment is available at SFO Medical Clinic (Map and Directions to SFO Medical Clinic (PDF)). Treatment is also available by the employee's personal physician only if a Pre-Designated Physician Form has been submitted and is on file with Human Resources at least 30 days in advance of the work-related injury or illness.
• Workers Compensation - Medical Treatment Form (PDF) - To be completed if the work-related injury required medical treatment.
• Workers Compensation - Declination of Medical Treatment Form (PDF) - To be completed if the work-related injury did not require medical treatment.
• Pre-Designated Physician Form (PDF) - Employees may designate their personal physician to handle Workers Compensation related injuries or illnesses. Treatment of a work-related injury or illness by the employee's personal physician is allowed only if a Pre-Designated Physician Form has been submitted and is on file with Human Resources at least 30 days in advance of the work-related injury.
